We are here to identify your child’s strengths and
needs and to create a family-centered plan to
address those needs, at no cost to you.
Your team
A team of people will identify your child’s strengths
and needs. This team will include:
Any time you have questions or concerns, please
let us know. Your comfort, understanding of the
process, and goals are important to providing the
best services for your child and family.
The process
Step 1: Evaluation
The evaluation is designed to be relaxed and
fun. Your child will participate in a variety of play
activities that relate to each area of development.
We would like to complete the evaluation in your
home, which is probably the most familiar place
to your child. However, if you are not comfortable
with that, we will work with you to find another
setting for the evaluation.
Step 2: Routines Based Interview (RBI)
The Routines Based Interview will allow us to visit
with you about your family’s daily routines, which
are important to your child’s development, as well
as any specific concerns you may have.
Step 3: Individualized Family Service Plan (IFSP)
Your team, including you, will gather to talk about
the results of the evaluation and the Routines
Based Interview. If your child shows a need for
services, then we will work with you to create a
plan that focuses on your child’s individual needs
as well as the needs of your entire family. This
type of planning is the best way to enhance your
child’s development.
Step 4: Start of Family-Centered Services
Your Family Service Coordinator will help you
find and use resources at the Child Development
Center and other community agencies to
meet your child’s needs. Your Family Service
Coordinator also will support you as you continue
to play the important role of your child’s first
teacher.

your child in learning through play.
